Please use WikiFormatting when writing in Trac.

install mysql5 +server

There is no such thing as a "server" variant in the mysql5 port. There was years ago, but not anymore. Now you can get what was in that variant by installing the separate port mysql5-server.

This was not a clean attempt. Please "sudo port clean mysql5", then try again, then attach the new main.log if it fails again.

You might also want to try the mysql51 or mysql55 port instead.
