Uninstalling mplayer doesn't actually remove any of the files:
Uninstalling mplayer 1.0rc2_0+aa+caca+dts+dv+faac+macosx+sdl+speex+theora+twolame+x264+xvid
When the update tries to activate, I get
---> Activating MPlayer 1.0rc2_2+aa+caca+darwin_8+dts+dv+faac+macosx+sdl+speex+theora+twolame+x264+xvid Error: Target org.macports.activate returned: Image error: /opt/local/bin/mencoder already exists and does not belong to a registered port. Unable to activate port MPlayer. Error: Status 1 encountered during processing.
When I force-activate the update, all the files are still there:
# port -f activate MPlayer ---> Activating MPlayer Warning: File /opt/local/bin/mencoder already exists. Moving to: /opt/local/bin/mencoder.mp_1203365349. ... and so on for several pages. Removing the files manually resolves the problem.
Seen on OSX 10.4.11 and 10.5.2.

Known bug, already filed as #11759.
As a workaround, you have to specify the exact casing.
