You are correct, I've got an enhancement planned but haven't had time to put it through. OTOH, users have to do several steps manually anyway, so this is just another small part.
You need to deactivate the conflicting version
sudo port deactivate qt4-mac-mysql5-plugin
You're also going to run into the same issue with (from memory) p5.16-dbd-mysql.

mythtv has recently switched from mysql to mariadb, so installed mythtv had mysql installed. updating should recognize this & do the right thing: uninstall mythtv, remove mysql, reinstall mythtv
